If that doesn't explain your issue, please check that the computer you're on is not using a vfat/fat32 file system. Anki currently is unable to detect changes to the media folder when it's on such a filesystem, so media syncing will not work in this case. A workaround is planned for a future release.

Adding audio to cards ("Basic" type) worked, but I wanted to go a step further by changing the card type to "Type in answer". Then the audio stopped working, both on my desktop (Mac) and iPhone app.

It's not a big deal having to click it though, but studying just flows better when the sound just plays automatically. On the iOS app on my iPad the same sound files that work as described previously don't play at all. Even with full volume and keyboard clicks on.. it's not that the sound is quiet or anything, it literally does not play.
