Tested VirtualBox 5.2.32 on Windows 7 host and Windows XP/7 guests. Audio quality is good. There is no distortion/skipping/stutter. However, delay is built up while playing sound. You can actually try disconnecting audio and reconnecting by going to devices --> Audio --> Uncheck Audio Output, then again check Audio Output. Try enabling audio output only and disable audio input. If it doesn't work, go to your guest machine and troubleshoot. If above steps doesn't work, Install Guest additions.
