To look in your system to turn off digital encoded output, you go to audio midi setup, and select the system audio and change it to the correct output. However this is usually automatic and won't stay on unless the hardware is in the socket. Which again brings me back to thinking it's a hardware issue and you should go to apple store immediately.

Just resolved this issue on OS X Mavericks. I'm not sure how it started on my computer to begin with. I went to Audio MIDI Setup. Then in the Audio Devices I went to Built-In Output. Then where it says "2ch-24bit Integer" I selected "2ch-16bit Integer". After that I was able to change the volume and the volume icon wasn't greyed out.

Surround Sound is shown as an option but it is grayed out and not selectable. All drivers are up to date as of four days ago, and I have the on-board sound of the motherboard disabled in its BIOS and it does not show up under Windows device manager, nor are there any other audio devices except the RealTek HD HDMI sound device.

I never needed MIDI through, and doubt interfaces have it. My sound modules do. I've used MOTU MIDI interfaces since about 1996. Never had a problem. I use a Mac so I just setup the devices and connections in Audio Midi Setup and the DAW sees them just fine and loads the patches if they are in the database.
