I work on similar task (virtual audio device on AVStream) and found one problem: my device is in the list of Windows playback devices, but AVStrMiniPinProcess is called only when I play sound by KS. In other case system is called AVStrMiniPinSetDeviceState to KSSTATE_RUN, but isn't AVStrMiniPinProcess.
