Start ManyCam and from the Video tab, select Cameras, and then click on the camera or video capture card you want to capture. Start Flash Media Live Encoder (or your choice of live encoder) and select 'Manycam Virtual Webcam' for 'Device' (or 'ManyCam video source' if you see this entry) The video feed should now show inside the FMLE preview window.

I'm using Stereo Mix for audio, as it gives me the closest results; Virtual Audio Capture is not syncing at all. Output video settings are : NTSC 60i and 30p. FMLE settings are for video are: VP6, 30.00, 1920x1080. By the end of a 90 minute stream, I'm about 150ms, 200ms out of syn, but perfectly fine at the beginning of the stream.
