Check to make sure there is +12V at the “Remote” connection of the amplifier. In some cases, the turn-on lead from the source unit is insufficient to turn on multiple devices and the use of a relay is required. To test for this problem, jump the “+12VDC” wire to the “Remote” terminal to see if the amplifier turns on.

Check your remote wire. Could have came loose from the H/U or on the amp. Also make sure your sub option on your H/U is ON. You can take a wire .. speaker wire, put it from the 12v input to the remote input. If the light turns on your problem is with the remote wire. If it doesn't it's with the amp it'sself. This really seems like a signal source problem. It's unlikely that the op-amps have failed. If you're 100% sure that the signal source isn't the problem, you could start troubleshooting by confirming that you have ±15v on pins 4 and 8 of the op-amps.
