You can tell ALSA 1,000 times to use the nVidia audio, and it says, "NOPE!" Once you get the Pulse control installed, look at the ALSA control and see which one says "Plugged in". On mine it's HDA2, and in Pulse the ones that are NOT used say "not plugged in". Go to the PLAYBACK tab at the top. At the bottom it defaults to APPLICATIONS.

Unfortunately, the kernel won't see the audio function unless you remove the graphics function first, which temporarily breaks the console. Because of that, it works best if you write a script to do it so you don't have to SSH into the system or try to type the commands into the console blind.

HDMI sound works with windows (running dual boot). I have the option to select HDMI output with pavucontrol, but no sound is produced.

You need to get an HDMI equipped PCI-E card to replace your integrated video and connect the soundcard or integrated audio to that card via an internal SPDIF adapter. What you are trying to do simply does not work because DVI ports do not carry audio signals.
