There is no real way to convert polyphony to midi accurately. There's overlapping tones/frequencies in every note so when you have a polyphonic recording there are so many variations it's very difficult to work with. That being said Melodyne is about the best I think you're gonna get at this point.

You normally always need to clean up the midi and it works best on clean audio of single instruments without effects (the drier the reverb the better).

I used it mostly in Sonar where its ARA integration means you can literally just drop an audio clip on a MIDI track and it will be replaced with a MIDI clip.
