1) Disable hardware graphics acceleration following steps mentioned below, then try playing the *.mp4 file in PowerPoint and check. Open PowerPoint >go to File >Option >Advanced >Display and check Disable hardware graphics acceleration > OK > OK. 2) You may also insert a *.wma file and check whether the issue re-occurs.

It seems that the video uploaded to the presentation is out of sync. What happens when you play the video without uploading to the PowerPoint slide, is the video and audio out of sync? I am a college professor teaching online courses, and for months I have used Powerpoint 2010 to create narrated video lectures. I create a powerpoint, upload the .wav file, record the timings of the animations and the slide transitions, and then use the save & send feature to export to .wmv. It has always worked out great.
