I have a Z9F soundbar connected to the TV connected via HDMI So I went through some more testing and it's actually an eARC issue (although the lastest tv update has helped somewhat with reducing audio dropouts). Turning off eARC fixes the missing audio at the start of videos and significantly reduces the occurrence of the audio drop outs.

I am also having the same audio drop-out issues for the last several weeks with my XBR65X950G connected via eARC to a Sony Z-9F. The sound will randomly cut out or go VERY low when viewing DVR, Netflix, or live Comcast Xfinity. If I rewind, the sound usually plays back fine. All cables were new as of Dec 2019, and all software is up to date.
