That's because spotify is not playing back over pulse, which is not what you want as that will block the audio device for everything but spotify and vice versa. The fuser output should return only pulseaudio even if you are playing something in spotify. And you should see at the very least a [alsa-plugin] spotify stream within pavucontrol.

Does Spotify require pulse audio? Especially of recent updates? Using the debian package repo a recent update broke audio. The error log seemed to indicate an issue with connecting to a service, and after installing PulseAudio it worked fine. My guess is that it requries PulseAudio but doesn't pull it in as a dependency. Basically (if I understand this right) you use PulseAudio to push the Spotify (running in Wine) stream through Icecast out to the network. I hit snag in the first section "Now, run Spotify as padsp wine spotify.exe. Spotify should pop up in your PulseAudio Volume Control (pavucontrol) in the Playback tab."
