I feed HDMI to my AVR which then feeds the TV via HDMI. I also can feed the optical to the AVR and use it for audio testing. You posted "video going direct to the TiVo" which is impossible. Some people feed HDMI to their TV then send the audio to an older receiver with no HDMI input. Sony does pass DD+ out its optical port.

When I disconnect the hdmi cable from the tv and leave it connected to the tivo its fine. As soon as i reconnect the hdmi cable to the tv, the audio starts cutting out. Very strange and very annoying. I also tried using the analog outputs by connecting the tivo via rca audio outs to a different input of the receiver.

To my experience, it does not. If you connect HDMI to my TiVo romio it loses the audio on the a/v 1/4 jack. In order to receive audio I needed to convert the optical out to rca left/right using a conversion box. I don’t think there is any lag between the video and audio when using the optical out, although I’ve never used it that way.
