Are you trying to produce two separate balanced outputs ? That can be achieved with normal two gang pots, one per channel. Otherwise you will need a four gang pot. These are available. I've just finished an Aleph P1.7 balanced pre-amp with an ALPS 4 gang pot controlling the volume. It's even easier if you look at switched ladder attenuators.

To bypass the volume use a short piece of wire on the pot pins. Counting down from the top of your picture, that would mean connect Pad 1 to 3 and 4 to 6.

A quick rule of thumb for selecting the resistance of a potentiometer is that you want the input impedance to be an order of magnitude (10 times) higher than the output (source) impedance. One thing that one does need to keep in mind is the issue of high frequency response. The input pot and the input (DC blocking) capacitor form a first order LPF.
