To get a debug log go to Settings->System->Hardware and turn on "Enable debug logging", then restart xbmc and play a video that has the issue you described. You can then find the debug log in your home directory in .xbmc/temp/xbmc.log .

THE VOICES ARE AT A FASTER SPEED THAN NORMAL. Someone with a normal voice sounds like a munchkin Open WMP 12; Open Now Playing mode by clicking the View menu tab and selecting the Now Playing option. Right-click the main part of the screen and then choose Enhancements > Play Speed Settings. Adjust from there accordingly.

Re: Audio/video playing too fast through HDMI. Perhaps a thing to try would be to set things to 48kHz. By default PulseAudio switches between 44.1kHz and 48kHz sometimes. Disabling this and making it only use 48kHz can be done by creating this file here: ~/.config/pulse/daemon.conf.
